For surgeons, podiatrists, and their authorized representatives

If you are a surgeon or podiatrist whose profile appears on TrustCut, this form allows you to (a) request a correction to your profile information, (b) submit a complaint about a specific rating you believe violates our Terms of Service, or (c) submit a written response for potential display alongside your profile.

Before You Submit

Profile information comes from the public NPPES database. Most inaccuracies can be addressed by updating your NPPES record directly at nppes.cms.hhs.gov.
Ratings are subjective opinions of individual nurses based on their firsthand professional experience. We will not remove a rating simply because you disagree with it.
We will review good-faith complaints that a rating is knowingly false, fabricated, submitted by someone with no firsthand experience, or otherwise in violation of our Terms. Removal is in our sole discretion.
Pre-suit notice is required. Under Section 10.4 of our Terms, you must provide written notice and allow at least 30 days for review before filing any lawsuit.
